DeForm is a web3-first form that opens up a whole new world of marketing, sales, and product feedback for companies, creators, and communities.

With these forms you can show off your ENS names, do on-chain lookups of ENS data (primary name, avatar, links, etc.), and even correlate ENS addresses with tons of other social identity and wallet verification data.

Some early DAOs are using DeForm to collect domains for sale. Others want to know that the market research feedback they're getting are from qualified ENS holders.

DeForm also allows you to perform any arbitrary ERC20, ERC721, ERC1155, and ENS lookup on any connected wallet on form responses. That means you can check for $ENS token holdings, POAPs, MintKudos NFTs, and a loooot more.

We're planning on adding a lot of integrations and making the data lookup functionality much easier to use. We'd love to work with the ENS community to get as much feedback and design iterations as possible :)
